Participant Consent Agreement

PARTICIPANT CONSENT FORM (MEDIA RELEASE FORM)

This Participant Consent Form (the "Form") is entered into by Perfect Pairing Events Inc., located in Ontario, Canada ("Company"), and the undersigned participant ("Participant").

By agreeing to this Form during event registration and purchasing a ticket, the Participant agrees to the terms outlined below regarding the use of photographs, videos, and audio recordings taken during the Company’s events, specifically cooking classes for singles at rented venues.

1. Consent to Use of Media
Participant hereby grants permission to Perfect Pairing Events Inc. to photograph, videotape, and record audio of the Participant during any event hosted by the Company. The Participant agrees that the Company may use, reproduce, and distribute this media in the following ways:

• Company Website: Display of media on the Company’s official website.

• Social Media: Posts, advertisements, and promotions on social media platforms such as Instagram, Facebook, TikTok, etc.

• Promotional Materials: Inclusion of media in flyers, ads, email newsletters, and other marketing/promotional materials.

• Press Releases & Media Publications: Use of media in press releases and news publications.

• Partner Collaborations: Sharing of media with third-party partners (e.g., venue owners, sponsors) for promotional purposes.

2. Public Sharing
Participant agrees that Perfect Pairing Events Inc. may share their media with third parties, including but not limited to sponsors, venue owners, media outlets, the cooking instructors and their catering companies, for the purposes of promotion and advertising.

3. Duration of Consent
This consent is granted indefinitely, unless revoked by the Participant in writing. The Participant may withdraw their consent at any time by contacting Perfect Pairing Events Inc. via email at info@perfectpairingevents.com. Upon receipt of the withdrawal request, the Company will comply within 14 days to cease further use of the Participant’s media. However, the Company cannot guarantee retrieval or removal of media that has already been used, published, or distributed.

4. No Compensation
The Participant acknowledges and agrees that they will receive no financial compensation or remuneration for the use of their image, likeness, or audio recordings in the media.

5. Opt-Out Clause
If the Participant does not wish to be photographed, videotaped, or recorded, they must notify Perfect Pairing Events Inc. via email at info@perfectpairingevents.com prior to the event. The Company will make reasonable efforts to accommodate the request and ensure that the Participant is not included in any media content.

6. Legal Agreement
By checking the agreement box during registration, Participant confirms that they have read and understood this Form and agree to the terms outlined herein. This electronic acknowledgment is legally binding and has the same effect as a handwritten signature.
